When you die, your soul will be put along with your body in your grave, and depending on where you're going(heaven or hell), you'll be shown your destination. On the Judgement Day, Allah will send the Muslims and true Christians and Jews(those before the coming of Islam) to heaven and the others to hell.

NO. Consciousness is just the brain working, no brain no consciousness. What we think of as consciousness is just the electrical signals jumping back and forth. I don't believe in a 'soul'.
