Here are the facts:
You must lose weight right away if you want to survive.
My brother had diabetes, congestive heart failure (cause by flu), and was overweight. He is dead.
The main thing is to lose the weight at about 3-5 lbs per week and no more than that. It is slow but for you and your health slow is better. You do not want to shock your system by loosing too much to fast.
Cut down on your calories. Yes the 2400 cal diet is good. The best is to cut out high calorie foods and drinks. Replace with lots of foods with low calorie, high fiber and low fat and low salt foods.
You should fill up with veggies, and salads and whole grains. Eat smaller portions of mean (about 4-6 oz) eat eggs for protein and only low fat cheese in moderation. Your main diet should be natural whole foods (most uncooked)
These foods help fill you up with less caloric content. Fiber does the trick to feel full. Eat 6-7 tiny meals per day and use a small salad plate for your meals (not a dinner plate). You keep your metabolism high that way and feel less hungry.
Drink only water and maybe milk. Becareful with sodium it is deadly too you. If you can get exercise just moving your legs and arms around do it. You need the exercise to keep the fluids from building up in your extremities. Put your feet up when you can.
Even take the wheel chair outside and down the block.
Your health is bad but you can make it better, it is your choice. The main factor is you must lose about 75 lbs to take the load off your heart and system. You will feel so much better and all your organs will thank you. You can do it. Just focus. And get all the bad things you eat out of the house.
Good luck

Oh my, that is a big problem, but not insurmountable. This forum is NOT, repeat NOT the place to be getting advice for this kind of thing. I should hope your doctor has gotten you in touch with a nutritionist who can help you understand what you need to do, as well as a physical therapist to help you safely increase your activity.
You need to pull out all the stops and insist on getting the help you need in person. The squeaky wheel gets oiled. Be the squeaky wheel, and first make an appointment with your doctor. Take notes. Ask him/her to help you develop a plan to get healthier mentally and physically. Insist he/she help you get connected into the services that can provide that help.
Best wishes, you know you can do this and you know you must do this. You KNOW you are so very worthy of a nice healthy life.

Here's the thing. With all those medical conditions you listed, you need to talk to your doctor before you change anything in your diet, especially if it is a drastic change. He/she may want you to make gradual changes so that it is not such a shock to your system. Good luck. It sounds like you have a good attitude and are on the right track.

You have too many health problems to be taking willy nilly advice from strangers onthe internet.
ASK YOUR DOCTOR.
If you have all of those health problems, I assume a doctor diagnosed you with them and provided you with presecriptions. Give them a call and set up an appointment to get yourself a diet plan that will work for you without risk.

Yes! Cut your intake and your heart will say thank you. As far as being Bipolar I'm afraid to give advice on that. Your doctor can help you with that. How about going on a diet of 1,300 per day and see what will happen. My wife lost 65 pounds going down to 1,300 per day. It took 10 months.

try weight watchers. I have tried them. And I think that this company does care about your weight, and they do provide many different services. I have lost weight on there support program. I do not recommend there program online. Going to meeting and speaking to a live group of people is best. Hang in there, things will start to look up.

My advice to you would be to go and see your doctor, and discuss a way forward with them, as they are best placed to know your situation and advise you.
