Regardless of whether he is actually trying to have sex with you, there are two meanings for this phrase that I can think of off hand.
1) Constipation (poor bloke) - though I don't know why he would discuss it with you. :-)
2) It's a sexual reference. He may not be trying to get you to "help him out" with his situation, but he may be trying to explain that he's gone too long without getting off. I suppose it depends on the context of your conversation.

Probably it means that they are trying to get you to belive that they really need to have sex, but it's a lie-- sex is not actually a necessity for anyone.
