my mother last name is on my birth certified but all my life I have been using my fathers last name now I need to know what do I need to get and birth certified

You probably have to go to the court house. You birth certificate has your mothers last name. So thats the name you need to give them. They will prob be able to find it that way. Not with the last name you have now. I'd go to the court house and ask them what you need. I would take in your drivers liscense for ID and your soc security card. They will prob ask for them. Contact the Hall of Records for the town you were born in... you will have to request the birth certificate in the name that is ON the birth certificate (you can do this on-line in most major cities). Once you have that document (that is your "legal" name until you have it changed either by marriage or by the court), you can take it and request a legal name change to whatever you like. In fact, you can order those forms on-line from the hall of records as well. If you already have a driver's license and social security in the "new" name, then you shouldn't have to worry about anything. Where it gets dicey is when you want a passport. You'll have to show proof, along with your birth certificate, that you've legally changed your name. But just getting a b/c should be as easy as using your b/c name to obtain it.

I had my father's last name on my birth certificate, but used my mother's maiden name from the time I was 5 until I was 18.

At 18 I went to court and had it legally changed to my mother's maiden name. You have to legally do this and it will cost about $150 to $200 and then you will have to go to the DMV and Social Security office to change it with them.

Its an easy process and takes about a month or two.

They don't issue you a "new" birth certificate, they give you a modified one. It will have your original name on it and then it will be crossed out and will have your new name printed above it.

You cannot have a social security card with your mother's name unless it is on your birth certificate. You can have your mother's name on your drivers license, but in the DMV system it will have your legal name and have your mother's name as an alias. So who ever wrote that above is wrong. And you need to be very careful about using two names when one isn't your legal name.

You have to use the one on your birth certificate, that is how they look them up. They'll ask for mothers maiden name, your fathers name, that is what they have on your record. You got a lot straightened out to to do they'll want to see your social security card and a form like a drivers license to identify yourself.

You send for your birth certificate useing your Mothers last name. It doesn't matter what you've used all of your life, only what was on the original certificate at birth, okay?

If you're in a position where you absolutely have to explain the discrepancy, get your name legally changed. Then submit the birth certificate along with the legal document.

It shouldnt matter, go to the county clerk or records office and give both names. They should still be able to pull up your birthcertificate.

One year I went with my mother so she could get my grandmothers certificate and we realized she was using a different last name than what was on her certificate justlike you. So bring your Social security card and ID and try to get it. It really shouldnt be a problem.

If you are of age you can get a legal name change in court and then they will give you another BC with that last name of it.

You can get the one that`s on your brith cert. as long as you have an i.d. with name on it.I think it depends on what state it`s in to.In time to come just be who you are or at lease be one of your selves all the time.
