My best guess would be Mycobacterium tuberculosis. Both C. immitis and H. capsulatum have the potential to cause chronic pulmonary disease. Also, chronic histoplasmosis can cause coin-shaped lesions that resemble tuberculosis.

Cocci and Histoplasma are lung infections that produce skin test reactivity. Often, these infections can mimic the Sx of TB.
