fail in what? i am assuming you are referring to exams.
the reason why people fail exams or courses is because of the lack of commitment and focus.

Possibilities are:
either the person does not understand the expectation,
or does not do what is expected,
or not capable to do what is expected.
The above applies to whether it is an exam in school or assignment at work or just plain life.
