Does everything go on, even things like strep throat? Are your medical records check ups, physicals, operations, etc? Are your medical records the papers doctors write on when they examine you?

Everything that you write down when you first go see your doctor, every note that is taken, every phone call you make to your doctor, every little thing goes into your medical records. It is kept private because of the HIPPA act.

by law, absolutely everything done for you by a medical professional has to go into your record.

they are the papers doctors write on...they are x-rays...surgical notations....medicine that is given.....physicals....everything you go to the doctor for is saved in your chart.....
