I think 'azeem is usually translated as Mighty. So the phrase would mean By Mighty God, or as we might say, By God Almighty. Not to be said lightly, you are really caling on God to demonstrate the truth of what you are saying.

Allah = God
Atheem = Great
It is an oath. Like when U say "cross my heart and hope to die", Muslims say: "I swear by Allah the Greatest....that I am saying the truth"
It is not literal, as 'wa' is translated into 'by'.
