In 1948, the United Nations divided Palestine into two parts - the Jewish nation of Israel and the Arab nation of Jordan. They used the Jordan river as the boundary between them.
Until that division, there was no separation, and the Jordan river was considered the central feature of Palestine. So in the older songs, any reference to the Jordan river was a reference to the Holy Land in general, not to the nation of Jordan.

When the Israelites crossed the Jordan River they took possession of the Promised Land.
Heaven is the Promised Land for Christians.
Jesus was baptized in the Jordan River.
Christians are initiated into the church and become adopted children of God through baptism.

It refers to the Jordan River. But many times "crossing the Jordan" refers to death and crossing over into Heaven. I'm not sure where this symbolism stems from but I know it, or at least the crossing the river symbol, is also used in the allegory "Pilgrim's Progress" which was written early on in the Reformation period.

In the bible when Moses led the ppl out of Israel, they had to cross the river Jordan to go into the "promised land" (Israel).
Now when ppl refer to Jordan, it is their going into the "promised land" (i.e. Heaven)
And I thought all that bible knowledge would be put to waste!
