Just tell her you would like to catch up and ask her if she wants to go for coffee or lunch or something.

First, think of some activity that you would both would like to do, dancing, bowling, hiking, biking,going to a museum or a play, etc. Then type:
Hello,NAME, I have been thinking about you lately and I was wondering if you would enjoy (whatever / let's say going to a play, tell her the name of the play where it is playing etc) with me on Saturday(the date). I am hoping you will say yes to the (play or whatever you pick) with me. I like to (hike, bike, etc whatever the case might be) If there are any of these activities that you would enjoy or prefer let me know. I remember you well and hope you are ready for a lovely evening with me. I have heard the play is very good. Let me know so we can make arrangement to set something up. Sincerely, (your name)

Hi how you doing? say that your doing just fine. Then casually say that you want to go to the movies and invite her with you.
Or you can say that you found a new restaurant and you want to go and check it out.

just ask her out to lunch. like : hey, maybe we should get together some time, have lunch or something.
and if she says yes, and lunch goes well, ask her out, but more direct this time ^_^
