It depends on what city you are in, in my city it is not illeagal to ride without a helmet, but in other counties close to mine it is and the police there will give you a fine or warning. Just where your helmet to be on the safe side.

no, but if ur under 14 and its a bike, then yes
i seriously dont see the point of a helmet though, u have like a one percent chance of falling on ur head

no. ithink it is only with a bicycle but u should still ride with a helmet
