i have heard that people tip their doormen of the buildings they work in once a month. then i've heard some people tipping once a year (holidays)

In a building where you live, you'll be going in and out everyday and are not expected to tip every time. I'd do something like $30 a month, that's a dollar a day and then be extra generous at Christmas.

Bellman: $5 per bag if in a five-star hotel. In a lesser hotel, $1 per bag.
Concierge: $5 if he helped you with an issue.
Doorman: $1 if he hails you a cab.

I would think 15% which is the geneal accepted tip at restaurants except one time I didn't have change so ended up tipping 30%.
