If they are hidden (surreptitious), yes.

If there is a legal precedent or company policy that indicates that signage is required, then probably no. Most companies, due to privacy acts, must post signage and have to have a written statement as to the actual use for the cameras, such as loss prevention. If, during the course of monitoring loss prevention they see something that contravenes workplace policies, then they have the right to act.

yep. The only place a company cannot film thier employees is the actual stall where the toilet is.

it probably varies by state, but where I live, they have to inform you if you are under surveillance
