I think you meant to say "a woman's maiden name". A woman's maiden name is simply her original surname before she got married. So if it's originally Paris Hilton, and she marries Shrek, her name becomes Paris Hilton Shrek. And Hilton would be her maiden name. My deepest condolences to Shrek if such a thing is to happen, though.
