If you are using the salutations, you use the husband's name; ie. Mr. & Mrs. John Smith. This is the "polite", formal form. If you wish to include the wife's first name, you omit the salutations; ie. John & Kelly Smith. However, this form is strictly informal. You can also use Mr. John Smith and Mrs. Kelly Smith, but that's a bit unruly and long.

Mr. John and Kelly Smith is an unacceptable form, as you are not saluting Kelly as formally as you are saluting John, and Mr. John and Mrs. Kelly Smith is also considered incorrect by most grammar experts.

Proper ettiquett, I believe, is still Mr. and Mrs. John Smith.
If you want to satisfy the Feminist Movement, use Mr. John and Ms. Kathy Smith. John and Kathy Smith is a little too informal unless you know each other well.

Formal - Mr & Mrs John Smith

Casual - John & Kathy Smith

the rules of etiquette are not as strict as they used to be. As a matter of fact, there aren't many rules left. It depends on how close you are to the people who will be receiving the note.
If these people are close to you, send it John and Kathy. If they are business associates in some manner, Mr & Mrs J Smith. Use your own discretion.

Hello an Good Day to you,
I would recommend the Mr & Mrs Smith, both feel it is thier letter an either can open it.
But do you know her- the wife or the husband better ? If you know that she'd like to see her name(most woman get giggles getting mail with the married name on it) - course this is void if they are not newly weds.
Or neither cares than go with the first.
But for the most part ---
