Permanent color penetrates the innermost layers of your hair and does not wash out, hence being called "permanent hair color".
Think of your hair like a pencil... the paint on the outside of that pencil is the outermost layer of your hair, under that is a layer of wood (the middle of your hair), and in the center of the pencil is lead (the core of your hair). When you color your hair with permanent color, it is absorbed into all three layers of your hair so it does not "wash out". The color absorbed in the outer layers (the paint on the pencil and the wood) will wash out over time causing the color to fade.
Before washing your hair a dozen times, seek advice from a licensed salon professional. Overwashing your hair will just make it dry by stripping the natural moisture of your hair, not remove the color. If a salon pro is too pricey, look for a Sally Beauty Supply store. They sell a simple color remover kit for around $13, in a smaller white & blue box. It has two small bottles in the kit and complete directions. DO NOT USE ANY COLOR REMOVER WITH PEROXIDE OR BLEACH! These are processes that must be done by a professional or you may damage your hair severely or cause hair loss.
Additionally, color will not remove another color. If it is not too dark, you may put a darker color over it. Putting a lighter color over it will not lighten the existing artificial color in your hair. This is a case where you truly need a professional. Trying to remove a color on your can result in orange or green hair... and you may end up paying much more to have that fixed!

Usually not, however it does have a tendency to fade over time. But by that time, your roots will probably begin to show and you'll want to redo the color anyway. There are preparations you can put on your hair before you wash it, that are said to help preserve the color. Keep your hair covered in the sun, as that will also fade the color. I've heard that red is the quickest color to fade.

I've been coloring my hair with permanent color for 43 years. It does not wash out, however, it does fade a bit. If you really hate the color you can either go darker or buy a color remover. Otherwise, you're stuck with it until it grows out.

Usually permanent hair color doesn't wash out, but when your hair has new growth then the color is coming out.

I know mine always does! And here's a tip: the more you wash your hair, especially with shampoo that is not made for color treated hair, the faster the hair color will wash out.

It doesn't wash out, since it's permanent.
But it CAN fade out.
For example, I had red highlights in my hair, but after several months, they faded to a more brownish color (with a slight red tint to it). I actually liked the color better after it faded, because before it looked too unnaturally red!

FIRST.......ALWAYS REMEMBER,.. IN HAIR DRESSING FIELD ANYTHING CALLED .PERMANENT IS NOT PERMANENT.
IT IS... LONG LASTING ONLY.
when we curl we call it PERMANENT.but curl stay for long time , not forever.
so when we use permanent color , they stay longer but washout eventually...
another thing to remember is that our hair always grow.......so we need touchups all the times on new growth....the time we touchup, we apply some color on previously color and comb them to revive it.
there are however some shampoo made to retain your color.
