Caffeine is a vasoconstrictor, which means it makes blood vessels smaller. Some kinds of headaches are related to vasoconstriction. My guess is that when you have caffeine on an empty stomach, it hits your blood stream fast enough to cause symptoms, whereas if you have food, it slows the absorption of the caffeine enough to avoid the headache. Good excuse to have that doughnut with your coffee...

That happens to me too, you need to have food to balance out the caffeine. Trust me, you'll notice a difference if you have something to eat. Take an advil if needed

That's from the caffeine, not the sugar. Eat some food to keep your blood sugar up, and wait for the caffeine to wear off.
