I know where (hosp./city) & when I was born, I have had a copy before, & am ultimately replacing my social security card & passport.

The copy of your certificate should be filed with the country in which you were born. Call the county as ask for birth records. You'll need to prove who you are but the county is only one who can give you a certified copy of the record of live birth.

i don't know about the passport but as for the birth certificate, go to the county court house in the county you were born in and ask what floor to go to for that certificate. you will get paperwork you'll need to fill out and a charge. i'm from pa and there it's $10.00 for a birth certificate. they take cash, money orders, and sometimes credit cards

My sister had to order one for a new passport as well. It normally takes about 2 weeks, it all depends what state I think. And you'll have to get something notarized to fax back. The website provides you with the form. Try this website:

The county clerk keeps all birth records. You have get it from there, and pay a small fee $2? usually. I do not know if you have to go in person, or can do it by mail.

contact the VITAL STATISTICS where you were born, the capital of the state in which you were born, if you don't know what it's listed under. try county records.
