I'm pretty easy going, and if you're in my home, you're likely a friend, who respects my things as much I as do. Any newcomers are guests, and it's up to how I conduct my own personal things to be of example. I can say that I appreciate many polite gestures, such as removing shoes before entry, (though I don't require this), throwing away trash, putting dishes in sink after drinking a beverage. I will extend every courtesy to make someone feel welcome, and I just like the same courtesy back. I'm not a maid though. My car... well don't be a pig and eat and spill stuff, but then again, it's not the end of the world if you do, and certainly not the end of our friendship. Nothing crazy has occurred to where I'd have to question someone's manners.
Restaurant, big issue here for me. Be nice to the staff, not belittling or rude. Dont' be so darn picky about onions, or whatever in the food, or tie up the staff's time about special orders, (going out to eat is a treat, not your prima donna moment), clear up trash around you, (I tend to bus my own table out of habit, grabbing everyone's dishes, straw paper wraps, napkins, putting them together). If you don't like something, give the kitchen the chance to fix it, and don't overly complain about the issue. Eat with mouth shut, and don't talk, it's gross to see ABC food. Keep the kidies at dinner earlier so adults don't have to deal with them climbing on the back end of booths at 10pm when their having after dinner drinks or socializing. Leave a decent tip for gosh sakes. Square up the check, (decide who will pay first, if check is split before) so that it's not such an awkward moment when the bill comes. I could go on and on about this subject, but my hands hurts. hehe.

What I consider impolite in my home:
I take care of my home, keep it clean organized etc. eg you come in with dirty shoes, you don't use a coaster when it is on my expensive oak table, my dogs are not allowed on the furniture but you call them to sit next to you. I invite you over for dinner, make you a steak to order, you give me a lecture on how I shouldn't put A-1 on my own steak because it ruins the flavor.
Car: Again with the dirty shoes in my clean car, asking to f ing smoke in my car, picking your face or biting your nails and spitting them out in my car, leaving your trash in my car.
Restaurant: Being rude to the service staff for little or no reason, bringing your crying / unruly children to dinner, inappropriate dress in public, picking your teeth, smoking (even in the smoking area).

The things that drive me totally nuts is when a person does not take off their shoes when they come into my home, light up a cigarette in my car without asking first if it's okay to smoke (NO IT IS NOT) and feels that the entire restaurant wants to listen to their conversation.

Use common sense. If you think it will offend don't do it. When in doubt ask the other person before you do it.
