I also had a home day care. You do not need anything fancy. I just used a spiral notebook and documented the date, time, how many children were in attendance and sign beside it. That's it. That is a document. Make sure that it says Fire drill on the top of the paper. Do these once a month. On another form do the same with Tornado Drills.

Or you could come up with a form on the computer. But the state does not require any particullar form. You just create your own.

What is required depends on what state you are in. Check with whatever state office governs home daycares. You may be able to make your own up, or they may have something for you to use. If you make your own, it most likely needs the Date, Time, Time to Evacuate everyone, How many people (children and staff listed separately) participated, and Who conducted the drill. The document must be signed by the person who conducted the drill or their supervisor if there is one.

Anyway, something along those lines will serve as a legal document in most states, unless your particular state has a form they want you to use.

I would check with them just to be sure of what you need.
