There are many causes depending on the condition. Some of the causes are: malnutrtion, parasites, insect bites, allergies, some systemic diseases, pollution, mechanical damage, poisonous plants, stress with lowered vitamin B, androgens, dehydration, heat damage, cold damage. There may be more I'm missing not sure, but there's definitely more than one cause.

It depends on what disease it is. Some skin diseases are a result of toxins saturating the body to the point that the skin tries to get rid of them. Your skin is the last line of defense after the kidneys and liver have been overloaded with toxins.
