If it has already been 3 years, I don't think you have anything to worry about.

Every tattoo that I have got never turned out that way. There is suppose to be some swelling and scarring but that is part of the healing process. It sounds to me like your brother may have some bruising or busted blood vessels (even more so than what should be). If they are worse than they should be then there will be more than likely ink poisoning. If that is the case and it is as bad as you describe the only thing you can do is go to the ER because he could end up loosing his arm or worse his life if he waits to long. I have a question; Was the tattoo artist sanitary? Did they open up everything out of the plastic in front of your brother and use fresh ink? If they were not and did not your brother might be able to get all of his medical expenses paid for by the tattoo parlor if he gets a good attorney.

The inks used are non-toxic. There is no need for you to worry. Tattoo parlors are under increasingly stringent guidelines from not only their associations and individual states, but the FDA & EPA as well. All inks sold in the United States (Which is where I am assuming you are located) are regulated by the FDA & EPA. Ink poisoning is a myth. It's created by ignorant parents to keep their offspring from drawing on themselves or getting tattoos.
Fisheress has a very good point. You're more likely to contract an illness from dirty needles than to get "ink poisoning".

The only thing you may have to worry about is "how sterile this tattoo parlor is". You may find yourself testing for Hepatitis C due to dirty needles. I suggest you be tested. It is a very simple blood test. Many people contract this silent disease from tattooing and don't even know it due to the fact the symthoms are what you would think is a virus, muscle aches, arthritis, run down feeling, nothing you would think is serious. PLEASE get tested. There is no vaccine available for Hep C although a treatment is now available if you test positive. I urge anyone who has gotten a tattoo, active w/more than one partner w/o protection or have shared needles or other drugs......PLEASE GET TESTED.
There are over a million people in the United States and more outside the States that are totally unaware of this silent killer.
Be well all!

Tattoos are a major risk factor for Hepatitis c transmission. Hepatitis has no symptoms until the liver damage progresses significantly, so you should get tested for this disease. There is a treatment for it, but you can cross that bridge when you come to it (if you test positive for it). I hope you don't need it. Hepatitis C is four times more prevalent than HIV, yet so few people know anything about it. There are actually more than 4 million people in the US affected by this disease. I know, I was one of them. I did the treatment for this and have been cleared of the virus since. Best wishes to you.
