Do a nasal flush several times a day when you have an active infection, twice a day when you feel one coming on, and once a day in the winter when healthy, if you're prone to them. It keeps the mucus membranes moist and gets the bacteria out of them. Much, much healthier than all these antibiotics (what if you needed them to save your life from something not otherwise curable someday?).

For the flush... 8 oz. body-temp water, plus 1/4 tsp baking soda, 1/4 tsp salt (I like to use organic and sea salt plus filtered water, but if all you have is regular, it will not hurt you). Flush it gently through alternate nostrils using a neti pot or a baby nasal aspirator (I use the latter). Lean over the sink. Lots of satisfying goop will come out, especially at first.

Even my kids do this now. It's a bit uncomfortable, but it will cure you, and sinus infections really bring you down.

You may have chronic sinusitis. There are hundreds of over the counter remedies available that will work to varying degrees. I have dealt with this for most of my life. Like I said, the meds work in varying degrees.
I have had surgery and yet, like you, when off meds for a short while an infection comes back. Finally I saw another specialist that helped me. I don't know if this will work for you, and it sounds pretty weird, but it works well for me. After clearing the initial infection I began to follow his directions.
I bought Bacitracin Zinc, an over the counter ointment, put a generous dollop on a cue tip and swabbed the nasal passages as far back as I could tolerate, in the morning and before bed at night. No more infections.
The Dr. was oriental, but has built a reputation for solutions to persistant problem cases, my General Practioner speaks highly of him. I don't know if this is something he learned while getting his license, or if it's an updated practice from his homeland (understandable, but very strong accent), at any rate it does work well for me.
It seems that the air at night was causing too much drying of my sinus tissues which would cause irritated and sometimes cracked tissues. By applying the Bscitracin Zinc ointment I was stopping the drying action. I also work in a very humid environment, so the daytime dose stopped the absorption during the day. I wouldn't recommend using another ointment, many have to much medicine in them, but this really worked for me.
I know, I know, it sounds like a load of old wives tales, that's why I checked his creds. It works as well as it sounds strange, and it sounds very strange. I was ready to try anything, tired of meds at double dose for twice as long as usual rx, so I gave it a try. If you are at that point, give it a try. It was a pleasant surprise for me, and might be for you too.
P.S. There is very little taste to it, so it's not as nasty as it sounds.
