why don,t you contact the peru embassy? i am sure they have an application form. further more contact the peru home affairs on their website and download an application form. just write peru home affairs and take it from there your self

If you are going as a tourist and your airplane ticket is round-trip you don't need a tourist visa, just your passport.
If you are going for businesses or to study you can check out the website of the Peruvian embassy.
