Depends on your situation. Are you changing name due to a marriage or divorce. If it is marriage. Take marriage Certificate to the Social Security Administration to have name changed. If it is due to a divorce and within the divorce papers you asked for name to be changed back to maiden name carry those divorce papers to Social Security....if you did not request that in the divorce papers then you need the assistance of an attorney....for any other reason for name change it will need to be done through the court system and Any papers that the court issues you granting your request for a name change needs to be carried to the Social Security Department 1st, then the drivers license division etc.
