That depends, do you have current coverage or are you looking to start a new plan. Most companies, depending on the state you live in, may offer a pre-existing exclusion for a set period of time. This means that the company will not pay any claims related to the pre-existing condition during that time.
Other companies may do what is called rate up the plan. This means that they will charge additional premium to cover the pre-existing condition. Because state laws regulating insurance differ from state to state, it's difficult to give an accurate answer.
As far as cost, do your home work. Check around. But remember, a quote is just an estimate of the cost. Only having an application underwritten will determine what the actual cost will be.
One thing more, you might consider doing what is considered as a trial application with the company. This information will be looked at by the underwriter's to determine if they would approve your application (subject to full underwriting verification). This will prevent you from having several rated or declined policies in your name. This information is reported to the MIB (medical Information Bureau) and is used by other companies during underwriting.

Depending on where you live, there may be a high-risk plan (CHIP or Comprehensive Health Insurance Plan) available to you. To be eligible, you might have to prove that you have been denied by private insurers (like Blue Cross) and that you either have no cobra (from your last employer) coverage, or that the cobra coverage would be more expensive than the high risk plan. Also, there may be a waiting period.
Talk to an agent and see what all of your options are.
You also could look for a job that provides insurance - many of those do not have pre-existing condition restrictions.
It also depends on the condition as to whether a private insurer will take you. I have diabetes, therefore private insurance rejected me. Luckily I found a job that has great insurance and will cover my diabetes.

You do still have options even if you have pre-existing medical conditions. Also, as was mentioned by another person, if you haven't had treatment for a condition for 6 months, you can be insured for it again under a "basic health insurance" plan. But that only applies to basic health insurance.
So, if you aren't looking at COBRA or a high risk pool or a public assistance plan, here are some options:
1. Use short-term health insurance during the required waiting period for most open enrolment plans. Under HIPAA law, the continuity of coverage rules then apply to the new insurance plan even though you were covered by a less expensive insurance policy during the waiting period. After the switch, your new plan will cover pre-existing conditions immediately.
2. Put family members on different plans even if you need open enrolment insurance. The best health plan for one family member with medical insurance problems is usually not the best choice for others who don't have pre-existing medical conditions.
3. "Basic Health Insurance", as I mentioned above, covers all pre-existing medical conditions after the policy has been in force for 6 months and accepts all applicants. This is a limited benefit plan that does not cover catastrophic costs and is not available in all states. You'll have to check with your state government to see if it is available in yours. While Basic Health Insurance won't cover everything, it also doesn't cost as much as other plans.
4. If out-of-pocket cash medical costs are high, join a health care discount plan. This is not insurance, but does reduce your out-of-pocket expense.
5. Be sure to check with the state insurance department for plans for applicants with lower income. You may qualify for better coverage at a lower cost.

Your best chance is to join a group health plan, such as one you get through work. If you were under a group health plan, you should be protected under the Health Insurance Portability and Accountability Act (HIPAA) in this circumstance. Preexisting conditions are valid till the time you have not taken any treatment for them in past 6 months..If thats the case, no health insurance company could deny you coverage..
I would suggest you to go to Wellpoint or Aetna or likes of big companies..Also, try combining coverages from same company such as home, dental etc and other coverages..

What city do you live in? Pa. has a program called Adult Basic. Im not sure if it covers pre existing. It is worth a try.
