They never question it during the initial onset because they are blissfully unaware that they are "crazy." When I had my initial onset of schizophrenia, I did not think once that my beliefs, visions, and hearing was out of whack. Not until I came out of it and could think clearly did I realize I was "crazy" during that time. Ever since I have been acutely aware of every tug back to insanity and yes, I question my sanity often, but I know if I can evaluate myself, I am still sane.

Sometimes it seems that way.

A person I know who is delusional just boarded a plane for another country to avoid the FBI. Prior to this co-workers and professors were out to get him. He firmly believes that he is sane while everyone around him finds him to be delusional. He's the only one who doesn't question his sanity.

Well, first of all, "crazy people" don't like to be called crazy. And secondly, there are so many different types of mental illness, your question can't possibly be answered. Some people know that something isn't right and they seek help. Others have severe forms of mental illness, rendering them unable to ascertain if there is a problem.

No, sometimes having a mental disorder creates even more stress for the person making things worse. Occasionally during an episode they may not be aware, but afterwards they are. Some diseases they're not aware that they have a mental problem but they do know something is wrong.

Untrue. Many are acutely aware of their problems, and during their more lucid times are much distressed by their situation.
