It is better translated to the English slang phrase "The main man" also if you were to ask in Spanish for a specific place the answer would be like "it is right there" or "Ahi mero" ( right there) or "Are you done?" the answer would be "ya mero" (almost there), also "the top of the top" is better translated to " el de mero arriba"

lol :D, Tayeb listen to this: As we know , Coca & Pepsi are using some singers for their promo , Pepsi got Amr diab & Elisa, Amr Diab was singing in Marina last summer , and he was singing "Raseef nemra khamsa" , then when he reach the part " we a3lan kakola " , So he sing it like that "we a3lan Pepsi" :D and he was laughing and he looked at us and make a sign with his hands meaning that they paid him and he cant sing to cocacola :D ,

Besides being "the top of the top", as noted above, I have also heard/seen it in reference to "the big boss"/"the head honcho". I believe this may be regional and perhaps specific to Mexico.
