First of all I would take the video games away until I was sure that he understood "where" to go to the bathroom. I would have also made him clean up his own mess...etc. That's gross, and I really can't believe that a 7 year old did this...he needs closer guidance from his parents (you included...you are a parent).

It would be a great excuse to toss the video game out the window (sorry son, seems to have gotten all wet, kinda smelled like pee.... any ideas???)
Seriously though, this could be as simple as what you have described, or a lot more complicated. Either way, I would not be letting him play that game anymore. It could be just as simple as he held it as long as he could and then did a duty behind the sofa (which was the direct result of wanting to not stop playing the game) Or it could be something deeper (there is a rather sorted study done on pre-pubescent boys and public urination.... has something to do with sexual arousal and without the ability to ejaculate.... well enough of that).... if it's that I definitely would want to get rid of that game too.
If either of the cousins was female, you may want to watch him for a while, otherwise, ditch the game and you will be fine.

I read the others and no one really had a good answer to this and i will admit, I do not have a sure fire answer. However, here are some suggestions to consider.
You know how if the dog pees on the carpet, you rub its nose in it. Well do the same to him, and no i don't mean rub his nose in it (even though you might want to). Just make him clean it up. and the rest of the house. Say that if he wants to make the house smell like a toilet then he can clean it all up.
You may also want to consider scaring him a little. Tell him something like...you'll put him in diapers if he does it again or something. If you want, you can put him back in them, but i don't recommend actually doing so unless he calls your bluff.
The last suggestion is just a good old fashioned butt whipping. Don't beat the life out of him, just get your point across.

I think you should teach him some ettiquetes, first of all by telling him this is wrong, you have to go to the toilet at any cost.
After he understands and agrees then ask him to clear the mess that he has made, tell him that is his punishment and if it happens again, then the punishment shall be much worse.
If it happens again you can come on the Yahoo! Answers again and tell us what to be done.

he should have to clean the floor and not be allowed to play video games for awhile; also explain that his behavior is unacceptable because a bathroom was near; not getting up to go to the bathroom and continuing to play should not be allowed and should not go without consequences

Explain to him right now that this behavior is unacceptable and that he better not do it again. Explain that if he does it again, you will punish him (it needs to be something severe - personally I would spank here, but a lot of people don't). Then, if it happens again, do not fail to follow through. Repeat as required to align his behavior.

I would make him clean it up first, and second talk to him and tell him that it is wrong. Tell him he can't play video games anymore if he can't use the restroom. They can push pause, so he could go.
