I am sure that they are, if just simply by adjusting contrasts and lightness a bit.
I wouldn't say most pictures are as most are taken by random people that don't have photoshop or don't care so much about it but the awesome ones are (most of the time anyway) taken by amateurs or pros and those people usually are perfectionists. Considering a simple photoshop manipulation can really improve a picture...why not using it?
Just go on www.deviantart.com and you will see lots of awesome pictures there. Most photographers admit to changing their pictures a bit with photoshop (or other), even if just slightly.

if by altered you mean a dust spot or some cropping, probably. I'm a photographer, and I can assure you that some of us come up with pretty spectacular photos straight out of the camera. just because photoshop is available doesn't mean we all use it for every photo. with a lot of luck (and skill and a good camera) landscapes especially can turn out really beautifully with no altering whatsoever.

I worked in design and layout for 3 years and only about 25% of pictures (digital or film) came to me unmodified. Just about every head, fashion or model shot is touched up.
