Tell him to get the whole posture thing sorted - Alexander Technique or any other sort of thing... physiotherapy, whatever

Has he been checked by a doctor? He should be concentrating on standing tall - that's very important. Maybe he feels too tall, or taller than his friends, so he slouches without realizing it? In any event, if his spine does actually curve, I would think a doctor could fit him for a brace to wear under his clothes for support and to help the bones ease into proper position. It's so important for your lungs and heart to function right, and if you're in a slouched position they can't.

He definitely needs to see a doctor, and perhaps will need a backbrace or even surgery to correct his back. It's best to go as soon as possible, because it'll only get worse as time progresses.

He really should see a doctor or maybe a chiropractor. I would say doctor first, from what you said about his shoulders: He might have a problem with his bones instead of his back.
