Yes. But you do it under the Data Protection Act, not the Freedom of Information Act.

(Data Protection Act is for personal records, Freedom of Information Act is only for general corporate information.)

You shouldn't have to invoke the act, your new dentist will contact your old one for your notes. If there is any aggro you write to your dentist stating that your applying for copies of all your paper and electronic records and your request forms a Subject Access Request under the Data Protection Act. Your dentist has the right to charge you £10 as his/her admin fee. If you don't hear within 30 days you can report him/her to the Data Commissioner who will enforce your request.

You as a patient have a right to your medical records. They may charge a small fee but nothing outrageous. You will have to fill out a form or write a letter. The easiest way to do it is to find a new dentist and let his office do a "Medical Records Request" It's generally easier for a medical office to get the records.

Normally they won't give them to you but ask for the info of the new dentist and then mail them onto them.

To hand over a persons medical records is not normally done as you would lose all history on what was done.

Sure. Just be ready to pay for copies. According to HIPAA, your dentist has 14 days to comply with your request.

I require a signed request from the patient before I ever forward records. I usually mail them to the patient so that if anyone sees them, it is 100% the patient's responsibility.

The records are property of the dentist. You can not ask for them, but you can ask for a copy of them.

yes and no...some dentists will not give them to you, but when you find a new dentist they can request them and a copy will be sent to the new dentists' office

Your medical records are your property....Your dental records are the dentist's property.

Ask them for a copy, they may do it for a small fee...but they don't have to.

he will make his own in any case.
