There could be several reasons.
My cats will not use the litter box if it has even one pile in it, either urine or poo. Or if there is either on the sides of the box.
Keep the litter box very clean, including the litter pan it's self.
Another thing I have done is put a large piece of plexie glass under the litter box, so the floor is covered about 21/2 feet around each side of the box, so if they do decide to use the floor, they end up on the plastic.
Hope this helps.

I recently inherited my girlfriend's 2 cats, who enjoyed urinating outside the litter box, presumably to mark their territory--and it would generally be on an article of clean clothing. It may sound a little harsh, but I'd take said piece of clothing, rub it in their face for a bit (but for feces that might be a little harsh--maybe just hold their head by it for a while so they can smell it) and tell them "no" and "bad kitty" and all that while doing it, and then PUT them in the litter box and keep them there for a while. It's been 4 weeks since we got the cats and for the past 3 there have been no incidents--but if you decide to do it, make sure you do it soon after the act so they know exactly what they did--if you wait too long, they'll forget what it is they're being punished for, and then they'll think you're just being cruel...but don't worry, if you get them soon, they'll know what they did. Good luck!

P.S. Be sure not to hurt them--just make sure they know what they did and that it was wrong!!

Cats want their privacy. Is the litter box in a private area? you might have to get a hooded litter box. Since you said it's clean than that is the only thing I can think off. Don't let it get too full. My cats poop outside of the box when it gets pretty full of poop.

She probably does it because she can.
Cats are finicky creatures and they usually don't need a reason to do anything they do.
Have you changed the litter brand at all recently? It might be her way of protesting.
Don't rub her face in her pee. If done too much, it could cause respiratory problems. Not to mention the fact that if you tick the cat off, you could have a nice little pile of poop on your pillow.

Try using 2 litter boxes. My cat used to poop in the planters and flower pots until I put another litter box out for him.

She is marking her territory, leaving her 'scent' on HER floor because she feels threatened by another pet of yours (or YOU) on some level.
Either she is testing your authority as master or she is nervous about her position in your household..
Power play or nervous tension....
