it generally just depends on your doctor. If you have insurance it will cost nothing or a minor copay. I have had 2 removed from my face ad you can't even see were they were any more. Its super easy

Depends on how big it is. They weigh it after it is removed and then charge by the gram. Big mole= $$$$$$$$$$$$$$$$$

Go to a skin specialist on this one. Some moles are not supposed to be removed as I understand it. Cost will probably vary from specialist to specialist.
