Well if he is having severe abdominal pain and is running a fever I would get him to the hospital as quick as you can.
Sometimes it can be a sign that the appendix are going, especially if he is running a fever.
There is also a chance that he may have the flu.
Don't mess around with it get him to a doctor like NOW before something bad happens. This could be very serious. Quit reading and go!!!!!

If the pain/ fever has not worsened in three days (allocated for answering the question on Yahoo answers), then the likelihood is that it is not serious. But if it has worsened, then I am sure you have already taken him to the hospital, preferably emergency care center.
Abdomen is divided into four quadrants. The right upper quadrant has Liver, right lower quadrant has Appendix and parts of Intestine, Two middle quadrants usually indicate left or right renal/ ureteric colic and / or intestinal ulceration/ infection as in food poisoning or Salmonella infection. Lower quadrants similarly have different significance attached to them. Presence of fever usually indicates underlying infection which can be due to a number of reasons from an inflamed appendix (Appendicitis) to a ruptured hernia or ulcer in the stomach or intestine; Tumors can present like that. Constipation and / or vomiting should be noted as added and helpful symptoms for the diagnosis of the pain but really, instead of asking here, best would be to go to a nearby doctor.
Hope he is doing fine as you read these answers.

Anything that accompanies a fever should be brought to a doctors attention. Could be a burst appendix.

How much do you want him to get better and not worse? Is this something you really want to get advice on a site like this, from complete strangers? I think not. Get him to a doctor.
