Since working conditions isn't one of the choices offered, I would have to say infectious disease, such as influenza.

Coronary heart disease.
People in the early 1900's ate things that totally clogged their arteries and blocked their hearts and with the hard manual labor most of them did,many died at an early age.
Rhuematic fever was another problem that added to the problem as Rhuematic fever damages the heart muscle.
Add that to their diet and most people in the 1900's were nothing more than a ticking time bomb.
