Talking too loud, burping, arms on the backs of the booths, being rude to the waitress, and being too messy. Keep your arms off the table too. No blowing of your nose either. Pretty much all it takes is common sense.

Things I've actually seen people doing at restaurants that I cringed at:
Picking nose, Clipping/filing fingernails, flossing teeth, picking at a zit.
Other things I hate at restaurants that too many people do:
Talk loudly on cell phones, talk on cell phones for the whole meal while their companion sits in silence. I can't stand it when parents take their kids out and let them run all over the restaurant while they do nothing. I understand that kids will be kids, but the parents need to discipline certain behaviors and think of the other customers, and yes, I do have a child so I know how it is.
And living in Europe this is something I can't stand, the restaurants don't have smoking and non/smoking sections. So we'll go and find a table away from smokers, and it never fails, someone will come sit at the next table and light up, even seeing that we have a child with us they can't show some restraint and respect. Okay, sorry for ranting.

Focus on polite manners instead, and that's what you'll notice.
Polite manners are being pleasant to the wait staff, eating gracefully, sitting up straight, and having a quiet and pleasant conversation.

I just hate to see women putting on make-up in public especially at a table in an eating establishment ....
This is something that should be done in private in my mind along with clipping nails and combing hair..

Elbows on table, talking with mouth full, talking on a cell phone (unless it's an emergancy), talking loudly, being rude to the waiters, etc...
I don't mind makeup, but don't comb/brush your hair. It's just disgusting.
