sounds like it's not being left to rise for long enough... ideally bread should be left to rise twice, and in between you should knock it down and knead lightly again (to knock the air out of it and make sure everything's distributed evenly)
when you're leaving it to rise, are you leaving it to rise in the bowl or in the tin?
if it's in the bowl, you should knock it down and only then put it in the tin, to leave to rise by double again.
if it's in the tin, maybe the temperature's too high?
(the rolls probably work better because they're smaller and rise easier when they're in the oven, even though they've probably not got enough air in them to start with... if you leave them to prove for longer, they'll be even lighter and fluffier.)

There can be a number of factors causing this. Do you allow your bread to rise for a second time after you have kneaded and its doubled in size?- Once you have left it to double, you should shape and leave to puff up again.
Sometimes too much salt or sugar causes problems.Have you baked it at the right temperature too? Last of all, make sure you HAVE kneaded it for as long as you should, because its one of the most important parts. Hope this helps!

if you are following the directions for kneading it, it sounds like the yeast you are using is either old, or the water wasn't warm enough to activate it. Old yeast will not activate. Too much flour, not enough rising time in a warm place, covered, or maybe you didn't add enough yeast.

Sounds like you aren't kneading enough,try folding over repeatedly whilst kneading to get air into the dough.

Knead it, shape it and then leave it for a while (about 30 mins) again before it goes in the oven. That should help.

You have to kneed it the right way so that it gets air into the dough. This helps it rise better and maakes it light and fluffy also.
