I hope that it helps you. However I looked up the affidavit of support and it looks like the ones they have listed are only for spouses, relatives, and employers. So maybe you should read the above link and double check that this is really required. However if you are here on a F-1 status and wish to apply for immigration then you should go to this page if your relative is sponsoring you... http://travel.state.gov/visa/immigrants/info/info_1335.html

Good luck to you!

If you want to just see an example of what it looks like then click this link for sponsors claiming as relative.

There are two different affidavits of support:

1) is to be used for an Immigrant visa application or for adjustment of status (green card). This is form I-864. It can be found here http://www.uscis.gov/graphics/formsfee/forms/i-864.htm

2) to use with a student visa application or some other non-immigrant visa application. This is a form I-134. Look here
http://www.uscis.gov/graphics/formsfee/forms/i-134.htm

Please note that these affidavits must be supported by certain documents (e.g. copies of the tex returns, etc -- read the instructions to the forms).

US student visas are not for immigration. They are non-immigrant visas; we expect those kids to take their US educations home to help improve their own countries. If a consular officer suspects that the student does not intend to return, the visa will be refused.
