D2780 is a 3/4 crown. Adding extra charges for all the things that go with delivering the service is a bogus way of overcoming low insurance reimbursement. It is a scam that some HMO participants use to bring their fee up to a level where they can stay in business. Some include "sterilization" charges.

THE ADA CODE INVOLVES COVERING THE PAYMENT TOWARDS EVERYTHING INVOLVING A 3/4 CROWN. THIS SHOULD COVER ANY SUPPLIES TO PREPARE THE TOOTH FOR THIS CROWN INCLUDING TEMPORIZING IT. SOME DENTISTS WILL TRY MORE TO THE PATIENT, KNOWING FULLY WELL THAT THE INSURANCE CO. WON'T PAY FOR IT. SOME CHARGES LIKE STERILIZATION ARE OBVIOUSLY BOGUS -- WHAT ARE THE OPTIONS? TO USE UNSTERILIZED INSTRUMENTS FOR A LOWER FEE? LOL! CONSIDER CHANGING IN DENTISTS TO SOMEONE THAT PROVIDES YOU WITH APPROPRIATE CARE.
