You can't tell from the information given. There are a million causes for both fatigue and weight loss--everything from more normal things, like stress, to serious illnesses.
His cholesterol and blood pressure are very high, though, so if he wants to prevent serious health problems (stroke, kidney failure, heart attack, etc.) he needs to see a doctor about that. While he's there, he can address the weight loss and fatigue.

I don't know exactly what those figures you quoted indicate, but depending on your brother's situation... it may be tell-tale signs of drug abuse...
Like I said, I'm not sure, but when I was in college I experimented with marijuana and as a result I lost a lot of weight in a short time and my eyes were always baggy.. clues could be dark circles around the eyes, slightly darker complexion and constant fatigue.
Maybe its best to confront your brother and ask him! or at least find out what he gets up to some nights...
