I am not positive about the laws in the Philippines, but in most cases changing a persons name back to their maiden name is done in court at the time of the divorce. If you want to change your name back while you are separated and not divorced that will most likely require a petition to the court. Not everyone changes their name when they are divorced.
I stand corrected, it appears that you can use your maiden name. Go to this link for more information: http://www.gov.ph/cat_foreignaffairs/

Check with your consulate, but you may need to have legal proof that you can use that name (there is an exclusivitity of name issue here)

No you are able to furnish the child what ever final call you decide on. in case you decide directly to furnish the child the fathers call you are able to, even nevertheless in case you decide on the child to have your final call and you're married inclusive of your husbands final call properly then by way of default the child has your husbands final call. even nevertheless while you're making plans to divorce and don't choose the child to have the two of their names you are able to furnish the child your maiden call and turn returned to that once the divorce.
