i want to use my maiden name in my passpor. is it possible? what are the needed requirements for that? thank you...im from Philippines

I am not positive about the laws in the Philippines, but in most cases changing a persons name back to their maiden name is done in court at the time of the divorce. If you want to change your name back while you are separated and not divorced that will most likely require a petition to the court. Not everyone changes their name when they are divorced.

Check with your consulate, but you may need to have legal proof that you can use that name (there is an exclusivitity of name issue here)
