It is called Halitosis. It is chronic bad breath. It doesn't mean you are nasty, talk to your doctor about it. There are remedies.

It's great that you floss every day. However, do you brush your tongue every time? That is very important for keeping clean breath. Your tongue traps TONS of food particles between the taste buds, gross! Brush it really well with your toothbrush, or get a tongue cleaner. Also, use an antiseptic mouthwash like Listerine or Crest Pro Health, if you don't already.
Another cause might be dehydration. It seems like hardly anyone gets their recommended 8 glasses per day, but it's SO important. Drink lots of water, eat crunchy fruit and vegetables all day long, and chew sugarless gum to keep your mouth hydrated and healthy.

Floss, brush your teeth, scrape your tongue, rinse with a mouthwash of your choice. Avoid foods that make bad breath, like onions and garlic. If none of that helps, consult your doctor or dentist to see where the smell is coming from.

The odors from garlic and onions enter your blood stream and then it takes a couple of DAYS before you breathe it all off. As you do this, you will continue to have bad breath. You can only use a mouthwash or breath mint (sugarless) as needed to mask the odor. I have read a lot of answers here saying to eat parsley to get rid of garlic breath, but I have no personal knowledge about that. Try it.

This may sound weird, but there is a thing called a tongue scraper. It scrapes all the "white stuff" off your tongue, which causes bad breath. I'm not sure where you get it, maybe your dentist. Also, maybe you need a new toothbrush. Try a different toothpaste.... but I don't think anyones breath is going to smell ok after eating. So you may not be able to help that....

You might have a small area in between teeth that traps food (like me). Ask the dentist or hygienist for small brushes you can push between the teeth--more effective than floss.
Chew sugar free gum, to help remove any food.

This is a problem I would say that 1st needs to be addressed by your doctor.
Then if he or she think it is for a dentist...then that's the 2nd option.
Bad breath is really from your stomach...Of course it can also be from teeth!...but not as often.
So do the doctor first, then dentist....My pastor ALWAYS has it, and I wish I could tell him...its from his diet.
I do not mean garlic and stuff....but he needs to go to the bathroom more.Some people do not go, as often...and food is
not being disposed of as Quickly...as it should.

Check your dentist to see if you have an absess.
If this is not the case; gargle some mouthwash and keep trying to work out what the problem is.
