Who could have prevented the accident?
A lot of people.
If the inherit design of the reactors was correct, it never would have happened.
If the test that was being performed was done correctly, it would never have happened.
If the supervision of the plant was worth a piss, it would never have happened.

Are you asking who prevented the Chernobyl accident? Because nobody prevented it. Thankfully, over time, the nuclear waste has been cleared away.

Still, there are lasting effects.
