The fields which I prefer are like the four corners of the world.
I like Maths but i am fond of sports. I like physics but also like information technology. I find it great to read articles on Einstien's relativity and quantum mechanics but like to play PC games. Plz suggest me a good course or a good site where i can decide or any other good advice.

Computer Science -- you could program for anything for anyone, even freelance your skills.

Sports tends not to be a career - unless you are outstanding at something, it's nearly impossible to make a living at it (and those careers tend to be over by age 30 with the exception of a few sports like golf.) Keep it up as your hobby.

Don't worry too much about "deciding" - you are likely to change your career path, statistically, 6-8 times in the course of your life. See if you can "shadow" someone who is doing work you like for a day or two - it's different DOING something than studying it!

You can take tests like the Myers-Briggs to get pointed at different fields too. Many schools offer this type of testing.

And find something that you like - for now. You can change course in the future if you find something else you love more!

I can't give a detailed advice, but I'll make a suggestion. Try to study for a job that will give you the most money. Others may tell you to do what you'll enjoy. I can tell you from my experience that all work pretty much sucks. But it gets worse when you realize that someone with less education will make more money than you because you're in a field that's not very well compensated.
If you're passionate about other interests, you can pursue them in your spare time, or minor in college as well. If things work out, then switch over.

Mathematics, Physics, IT. Don't you detect a common thread here? Study any of these with Maths or Physics as majors or opt for IT engineering which also involves the other 2 subjects in varying degrees. Your love for reading about Einstien's theories and playing PC games are in keeping with your general academic preferences. And as for your other passion - sports, you can indulge in it in your leisure or as part of your daily exercises. Your work in science or computers can also involve sports dynamics.

Sports medicine sounds the way to go. Either as a doctor or physio particularly on the research side.

That would involve all of the above in varying degrees.
