I like a thin crust; a bit of chewy, but definitely some crispy.
Flavourful sauce, not overdone so the topping is sliding off. It helps to layer carefully; I've even gone so far as to stir some cheese into the sauce before spreading it on.
Mushrooms, onions, and green peppers, though sometimes plain is just right. Occasionally, any of: artichoke hearts, plum tomatoes, black olives, asparagus, hot peppers, feta.
Top-notch cheese; see http://answers.yahoo.com/question/index?qid=20061027092235AAihVLO&pa=FYd1D2bwHTHwIrtvEuw6RDCAT8YMAO.fmF6WvjmWNJriKx8t0EFNSnXdm7wir79ZGT23C359oI4x6w--&msgr_status=
It should be just slightly browned in places on the top, and not applied with a stingy hand.
Sometimes, though, the best pizza is sort of disgusting. Mr Kmennie is big on double cheese, pineapples, and green olives. I now and then get one with tomatoes, jalapenos, black olives, and nacho chips.
In a licensed restaurant where pizza's the specialty (but hardly ever a chain) with decent wine or beer, or at home from your own oven with milk. There's a lot to be said for somebody showing up at your door with something steaming hot in a cardboard box, though.
The best side dish is a simple Greek salad, though a small dish of antipasto has its place.
All situations are improved by pizza. I had it served (with other things) at a formal wedding, going on the idea that it's universally liked; there are reasons why 'Cornish game hens' are not routinely delivered to doors in boxes. That said, television and movies are especially more enjoyable with pizza, and so are family get-togethers.

Use half white wine half milk + flour + salt for the dough.
Bake it very thin and wide on baking paper.
Hot oven preheated at max!!!
Half-ways through (when it looks dry but still white), add some slices of tomato, some olive oil, salt, dry oregano, or some few table spoons of tomato sauce. Back in the oven and out before the edges turn black take it out of the oven. DON'T LEAVE THE ROOM until it is baked, because it takes few minutes and just waits for you to turn your head to turn into a piece of coal!
Eat it without waiting!
Let the people wait at the table until it is ready. Cold pizza is a nightmare for us Italians!

Thin-crust pizza with cheese, pineapple, pepperoni and ham. Absolutely delicious. Potato pizza's not bad either.

Generous portions of black olives, onions, mushrooms, extra cheese, sausage, and Canadian bacon make a great pizza.
Midnight (cold from the regrigerator) snacks make pizza that much better.
