A fissure is indeed a crack in any substance.

In relation to your shoulder, there were probably fissures or cracks in the cartilage overlying the joint surfaces. This can sometimes cause pain. I assume you had some sort of arthroscopic (keyhole) operation to smooth down the cartilage.
