You have to understand that a woman doesn't see through man's eye but her own. Women as well as men can only perceive what they feel are of the most importance to them or to other people. Sometimes she is looking for you to say something in return remarking on how skinny she is. This gives her a compliment and at the same time reassurance that she isn't fat. One line that has always seemed to be good for me is " I'm sorry but you look fine to me and furthermore I am more interested in what you have to say more so than physical appearance."

I am a woman. I do at times think I'm fat, but I try not to ask a guy specifically "do you think I"m fat?" because it puts them in an akward position.
