Social anxiety disorder seems like an obvious one. Depression is often associated with social withdrawl, but that's usually a change in behavior rather than having always been a loner. ADHD can often lead to trouble making or keeping friends, or maintaning relationships, but I'm not sure I'd quite describe that as being a "loner" exacty. Virtually an disorder can lead to insecurities that might cause a person to withdraw from social interactions and become a loner.
Of course, someone can also just be shy or introvered without having any disorder at all.

I am a special education teacher and see much of this. I've seen it in children experiencing an emotional or behavioral disorder, depression. The characteristic I have unfortunately seen the most is home life. Depending on the age of the person of course, the way they were raised can make a huge difference here. If they were told over and over again to shut up, or their presence was not appreciated or worse, then they eventually and gradually can become a loner. If they don't speak to anyone or get in anyone's way, there is no harm of anyone telling them to shut up or go away. Hope this helps. It's not medically helpful of course, but maybe it will help somehow.

There doesn't necessarily have to be a disorder of any kind for a person to be a loner. It could simply be a matter of personal choice because some people actually enjoy the sweet pleasures of solitude.
