Well, first of all, if he/she is at high risk, he should be getting medical care now! And--have a cardiac workup. There are medical interventions and medications that could help. For a person with a time bomb like that inside them, they should not be participating in strenuous exercise until they are cleared medically to do so. AND---You need to take a CPR class. You do not do CPR on a person who is still breathing and has a pulse! Furthermore, you alone cannot do CPR for 2 hours! Have you ever performed it?. It zaps all your energy quickly. If I really cared about my friend, I would not go along with an activity that might kill them and tell them I will not be a party to his/her suicide!

Well I would think you would and SHOULD take nitroglicerin with you....that's something your friend should be asking his cardiologist so that he is prepared with any of the appropriate medications he might need.
The nitro is all I know of that might help if one has a heart attack. I don't know that aspirin would do much in an emergency. If one is having a heart attack where there is a blood clot, that blood clot, if not dissolved, will block the artery and make the surrounding tissue of the heart die, and thus it's the damage, and amount of damage, that the clot causes, that destroys or damages the heart. If of course arteries break then there is nothing you can do. If his heart stops then you have to try to restart it, and I mean I can't imagine what more you can do really, than all these measures.

If he gets a heart attack (severe chest pain, High blood pressure, sweating, cold clammy hands etc) put him in a comfortable position like sitting down or lying down on his left side. Loosen his belt and make him more comfortable. If you carry meds for hypertension then immediately administer sublingually for faster results. CPR and chest compressions are done only when he stops breathing. Of course call for help right away.

Surely if your friend is at high risk of a heart attack, he wouldn't be hiking would he? Just make the patient comfortable - coma position etc until help arrives. If his hearts stops, then you will need to perform CPR.

CPR only if the heart stops keep him quite feed him aspirin and nitro tablets place one in under his tongue and repeat three times until you get response if no pulse do CPR make sure you know where he keeps his nitro tabs
